Here is the central location for youth forms. Any event that occurs off-site and involves us transporting students requires us to have these on file.  Below you will find two separate forms. The first form is a detailed medical and travel release. Though it is lengthy it is valid for a period of one year. The second form is much simpler and is nothing more than permission to participate in a specific event. We are required to have this paperwork in the event of an emergency so please take the time to fill them out completely. We have tried to make this as simple for you as possible. Please take the time to fill them out completely and accurately.

• Medical Release - This form is necessary for all off-site youth functions. The good part is that it’s valid for one year from the date of signature. Please fill this out completely. If you’re not sure your student has a valid one on file then contact Ron Hickson.


• Event Permission - This form is necessary for all off-site events. One is needed for each event and for each student.

You will need Adobe Acrobat Reader to open the files. Left-click them to open them in a new window or you may right-click and choose “Save Target As” to save the form to your computer. Once you open the file you can enter all of the information prior to printing.
